What to Expect

Senior Care By Design is a San Diego based, locally owned and operated service providing housing options for older adults. I’m committed to working with you to locate living options that include; independent, assisted living, memory and continuing care, and Alzheimer’s communities with the ability to meet your social and medical needs, location and budget.

Our Journey

Senior Care by Design identifies licensed assisted living and memory care communities that meet your family’s requirements. We will walk this journey together with grace & ease.

Step 1

INITIAL CONTACT

We begin the process with an initial phone consultation.  My goal will be to understand your family, their unique situation, and the type of assistance you are seeking.

Step 2

GETTING TO KNOW YOU

You’ll then complete an assessment that helps me understand your specific needs. I look at all aspects of care, including:

Personal Preferences:
Personal preferences can make all the difference when it comes to making the final decision as to which community is the best fit.

Based on your preferences, we will accompany you on site tours at communities you select from our recommendations. We provide one-on-one support and guidance throughout the decision-making process and physical transition into the assisted living community that you choose.  We are by your side every step of the way.

Some Lifestyle Considerations:

  • Male to Female Ratios

  • Religious and/or Ethnic Make Up of the Community

  • Pet Friendly

  • Availability of Off-Site Shuttle Services

  • Transportation to and from Medical Appointments

  • Access to Social and Community Activities

  • Unique food choices like organic fruits and vegetables

Level of Medical Care:
The level of medical care that a community provides is determined by the license issued by the state along with the policies set by the individual communities.

Some Healthcare Considerations:

  • On-Site Rehabilitation Services

  • Licensed/Certified Medical Staff

  • Designated Staff Physician

  • Dementia/Alzheimer’s Specialized Communities

  • Staff that will be available on a 7/24 Basis

  • Secured & Non Secured Environments

Finances:
We know financial considerations are important to you – and our experience can help you more easily and quickly evaluate your options and those available to you.

Veterans’ Aid and Attendance Program– A Program for wartime Veterans and their spouses. The Aid and Attendance (A&A) Pension provides financial assistance for those who require the regular assistance from health care workers with Activities of Daily Living (ADLs).  Senior Care by Design can put you in direct contact with the Veteran’s Service Officer in your region and other resources that can offer additional assistance in applying for this benefit.

Long Term Care Insurance– Long-Term Care insurance has expanded from simple nursing home coverage to covering care in assisted living facilities and in an individual’s own home.

Reverse Mortgages– For couples who are trying to finance assisted living care for one spouse while the other remains at home; a reverse mortgage can be a viable option. Available to homeowners who are 62 or older, a reverse mortgage lets you convert some of the equity in your home into cash.

Elder Care Attorneys– Elder law encompasses all aspects of planning for aging, illness, and incapacity. Powers of Attorney, Wills, Fiduciary Representation, Asset Protection and Guardianship are a few examples of legal issues that our clients may require assistance with.

Real Estate Agents –If you need to finance Assisted Living with the proceeds of a home, a good Real Estate Agent is essential. In addition to having the ability to provide an accurate market analysis (required by the Assisted Living Community), the agents we work with have been responsive to preparing homes for a quicker sale. In addition to the asking price, the agent’s ability to stage a property will be an important factor in how desirable the home is.

Step 3

EXPLORATION

Next we will meet to help determine if an assisted living environment is best.  Once we have discussed the medical, social, financial and location guidelines, we will compare living options.  Based on information gathered, I will either identify communities that meet your needs or recommend other options and we will navigate the process together.

  • Identify communities that may be a fit using criteria from Intake/Assessment

    • Budget

    • Geography

    • Timeline

  • Share potential communities with you and your family

  • Evaluate options and your feedback to find the best fit for your family

  • Visit communities & finalize arrangement

Step 4

FIND YOUR COMMUNITY

I work with many types of communities, which you can read more about here. We will find the right fit for a community where you will thrive!
